  • Take a printout of the auto-generated filled form.
  • Report on 20/11/2025 between 09:00 AM to 11:00 AM.
  • Venue: Conference Hall, 3rd Floor, Pt. Madan Mohan Malaviya Hospital, Malviya Nagar, New Delhi-110017.
  • Bring all original documents + self-attested photocopies + 5 passport size photos.
